We sincerely welcome you as our patient and are pleased that you are choosing Ville Platte Medical Center.  We are committed to making your hospital visit as comfortable and convenient as possible from beginning to end.  Our Registration Department, located near the front hospital lobby, is open:

Monday thru Friday

6:30am - 4:30pm

After hours, all registrations are facilitated through our  Emergency Department  located in the rear of the facility.

When registering, you may be required to sign the following:

• Consent for treatment
• Authorization for release of information to your insurance provider

If you are a Medicare recipient, you will also be required to sign:

• Medicare Secondary questionnaire
• Medicare Rights Statement

Remember to bring the following with you for registration:

• Social Security card
• All active insurance cards
• Picture identification
